Baseline data will be established for the JCAHO and CMS measures from the publicly available data
on the appropriate Web sites. Hospitals will be required to select participation in either the
New Jersey Infection Prevention Partnership or the IHI 100,000 Lives Campaign to benefit under this
indicator. If the hospital selects the IHI 100,000 Lives Campaign, the hospital must submit baseline
data to Horizon BCBSNJ with an attestation to the accuracy of the data.

Horizon BCBSNJ will collate the baseline data into an initial report to the hospital for verification.
The report will include the hospital's current ranking for the administrative measures included in the
program. The hospital will need to supply the percentile achieved in its latest patient satisfaction
survey. It will be from this baseline data that improvement will be measured. Recognition for patient
safety will be based on compliance with the JCAHO National Patient Safety Goals and improvement in
the data extracted from the New Jersey Infection Prevention Partnership or the IHI 100,000 Lives
Campaign as appropriate. Recognition for clinical outcomes will be based on the individual
hospital's performance against the New Jersey and national averages. Recognition for patient
satisfaction will be based on the percentile rating of the individual hospital's patient satisfaction
survey tool. Recognition for administrative measures will be based on the individual hospital's
ranking against the Horizon BCBSNJ participating hospital network.
