Members with Chronic Kidney Disease (CKD) or End Stage Renal Disease (ESRD) are eligible to join the CKD Health Education Program, which connects members with professional care specialists who help coordinate their care, provide information, and offer personalized support.

Participants enrolled in the program will receive:

• Health information related to CKD and ESRD.
• Important information about how to help slow down the progression of CKD.
• Personalized support for the care and management of dialysis therapies and treatments.
• Educational materials that reinforce clinical recommendations.
• Assessment tools to help monitor their progress.
• Tools to help them communicate better with health care professionals.
