Members, ages 18 and older, with diabetes can join our new program, which complements their doctor's care. All participants have access to professional care specialists and educational materials to help them better understand and manage their conditions.

Horizon BCBSNJ can help program participants understand:

• The diabetes medications prescribed by their doctors.
• How to properly monitor blood glucose levels.
• The appropriate tests and screenings needed.
• The importance of good nutrition and exercise.
