Horizon HMO members with multiple sclerosis are eligible to join the Multiple Sclerosis Health Education Program. The program centers around a team of highly trained professional care specialists who can help reinforce the plan of care established between the physician and patient.

Participants enrolled in the program will receive:

• Ongoing clinical assessments to monitor the use of medications and treatment plans.
• Educational materials that reinforce clinical recommendations.
• Assessment tools to help monitor their progress.
• Tools to help them communicate better with health care professionals.
