Horizon BCBSNJ filed an application on August 15, 2008, with the State of New Jersey to convert from a health service corporation to a domestic stock insurer.  In accordance with the state's conversion law on October 14, 2008, the Commissioner of Banking and Insurance and the New Jersey Attorney General deemed the application incomplete and posed questions to Horizon BCBSNJ about its application for conversion and plan for the creation of an independent foundation required by the conversion statute.
