Health Center Initiative

The Horizon Foundation for New Jersey Health Center Initiative was created to expand access to health care for thousands of uninsured and underserved individuals throughout New Jersey. This $5 million, five-year grant Initiative provides needed resources to health centers across New Jersey. The Initiative funds charitable 501(c)(3) Federally Qualified Health Centers and independent health centers throughout the state that offer comprehensive primary health care services. The Initiative does not fund
hospital-based (owned or operated) health centers or ambulatory care centers.

One of the priorities of the Foundation is to expand access to primary health care for uninsured and underserved New Jersey residents. In keeping with this priority, The Horizon Foundation for New Jersey Health Center Initiative was developed. For five years, beginning in 2006, the Foundation will award approximately $1 million each year, in total grants, to selected health centers across the state.

The goals of the Initiative are as follows:

  • Increase access to health care services for uninsured and underserved individuals
  • Provide resources to enhance health center operations
  • Expand services to health center patients

Grants range from $25,000 to $100,000 annually and can support a health center for up to five years in duration. The maximum award to an individual center is $500,000 distributed in equal grant awards over five years.

Applicants may apply for either annual or multi-year grants. Additionally, applicants may apply jointly for funding.

Past grantees are not automatically eligible to receive additional funding for a current project.

Funding Categories

Grant funds may be used for one, or a combination, of the following:

  • New staff positions
  • Staff development
  • Development of a specialty provider network
  • Mental health and substance abuse services
  • Purchase of office equipment (e.g., computers)
  • Purchase of medical equipment (e.g., X-ray machines)
  • Renovations (e.g., construction of an additional examination room)
  • Community outreach and marketing
  • Medication and lab fees
  • Translation services
  • Oral health and wellness programs

Proposal Review

An independent, volunteer committee, comprised of individuals from across New Jersey who understand the health care issues facing our state, make the funding recommendations to the Foundation's Board of Directors.

Please note: The Horizon Foundation for New Jersey reserves the right to accept or reject any proposal, for any reason, with or without explanation. The Foundation will reject a proposal if its contents are inconsistent with the Initiative's guidelines or general Foundation principles.

Funds Available

The Health Center Initiative is a $5 million, five-year grant program. In years one and two, a total of $2,316,080 was awarded. For the remaining years, $2,683,920 will be awarded as follows:

  • $   683,920 for year three (remaining grant resources for year three)
  • $1,000,000 for year four
  • $1,000,000 for year five
