Horizon Healthcare of New Jersey, Inc. offers a Medicare Advantage Special Needs Plan designed to help coordinate care for New Jersey residents who receive Medicaid from the state. Eligibility for this plan and the exact benefits available vary depending on your level of Medicaid eligibility. However, this plan provides all the benefits Medicare offers and more.

Comprehensive Coverage
Horizon Healthcare of New Jersey, Inc. has a contract with the Centers for Medicare and Medicaid Services (CMS) that allows us to offer you your Medicare benefits through our Medicare Advantage Special Needs Plan - Horizon Medicare Blue Solutions. In addition to our contract with CMS, Horizon Healthcare of New Jersey, Inc. has a contract with New Jersey's Division of Medical Assistance and Health Services to offer Horizon Medicare Blue Solutions. This plan includes:

• More benefits than Original Medicare
• Low or no out-of-pocket costs
• A large network of doctors
• Prescription drug coverage with government subsidies under Medicare Part D (as long as you continue to pay your Medicare Part B premium)
• Health and wellness education including a nurse hotline and health club membership
• No restrictions on when you can enroll and disenroll

Coordination of Care
This Medicare Advantage Special Needs Plan is designed to streamline Medicare and Medicaid benefits, making it easier to navigate the health care system.

In addition to your Horizon Medicare Blue Solutions ID card, you should also carry your Medicaid Identification card. You will need this card to access certain services not covered by Medicare but which are covered by Medicaid.

And you'll have peace of mind knowing that your health care will be coordinated for you by your personal primary care physician whom you choose from our extensive network of physicians.

Eligibility
To be eligible to join Horizon Medicare Blue Solutions, you must live in New Jersey and be entitled to Medicare Part A and enrolled in Medicare Part B. You must also receive Medicaid from the state.
