Horizon Healthcare of New Jersey, Inc. has a contract with the Centers for Medicare and Medicaid Services (CMS) that allows us to offer you your Medicare benefits through our Medicare Advantage (MA) plans: Horizon Medicare Blue Value and Horizon Medicare Blue Access. Through this contract, Medicare pays us a set amount each month to subsidize your premium thereby allowing us to offer you a plan with a $0 health plan premium for you to pay when you choose Horizon Medicare Blue Value w/ Rx Standard.
