Horizon Medicare Blue Rx Standard (PDP)
Featuring Easy-to-Budget Prescription Drug Copays and Savings of 50% on Generic Drug Copays with our Mail Order Service With the Horizon Medicare Blue Rx Standard (PDP) Plan option, coverage begins after you meet your $310 deductible. The Horizon Medicare Blue Rx Standard (PDP) Plan features a copay of just $8 for generic drugs, $36 copay for covered preferred brand drugs, and $72 copay for non-preferred brand drugs until your total annual prescription costs reach $2,830. You pay 100% of the annual costs you incur between $2,830 and $4,550 (out-of pocket maximum); however, Horizon Blue Cross Blue Shield of New Jersey's special negotiated rates for prescription drugs apply. Once your annual drug costs exceed $4,550, you pay just $2.50 per generic/preferred or $6.30 for all others or 5% of the drug's cost, whichever is greater.

Featuring No Deductible and Savings of 50% on Generic Drug Copays with our Mail Order Service The Horizon Medicare Blue Rx Enhanced (PDP) Plan option features no deductible and a copay of $8 for covered generic drugs, a $36 copay for covered preferred brand drugs, and a $72 copay for non-preferred brand drugs until your total annual prescription costs reach $2,830. You then pay $8 for generic drugs and 100% of the costs for all other drugs when your annual drug costs are between $2,830 and $4,550 (out-of-pocket maximum).
