Post-Discharge Contact Program
This program is designed to help identify problems or potential complications that may arise following an acute-care facility admission. A Nurse Case Manager helps coordinate services for members with identified needs. The Nurse Case Manager: - contacts select members who are discharged from hospitals;
- assists each member's physician in obtaining authorization for services (when required); and
- communicates to each member's physician certain needs identified or expressed by the member.
If you are a Horizon BCBSNJ Managed Care Member who needs additional services following discharge, please speak with your treating physician and have him or her call the appropriate prior authorization number. The Post-Discharge Contact Enhanced Case Management Program is another way we are Making Healthcare Work®' for our members and providers. The program will continue to help providers identify members who need additional health care services and keep members informed about their health conditions.
