This program is designed to assist members who need or already receive treatment for complex wounds.

For complex wounds, the Nurse Case Manager:

• works with treating physicians to facilitate access to quality health care in the most appropriate setting;
• helps to follow up with members undergoing wound care treatment;
• assists in the monitoring of the wound healing process; and
• provides general education to members and physicians as requested.

If you are a Horizon BCBSNJ Managed Care Member who needs services, please speak with your treating physician and have him or her call the appropriate prior authorization number. The Wound Care Enhanced Case Management Program is another way we are Making Healthcare Work^®' for our members and providers.
