Twenty-six million Americans have Chronic Kidney Disease (CKD) and another 20 million more are at increased risk for the disease.  Early detection can help prevent the progression from kidney disease to kidney failure.  CKD may be caused by diabetes, high blood pressure and other disorders.  Early detection and treatment can often keep chronic kidney disease from getting worse.*

 

Horizon Blue Cross Blue Shield of New Jersey empowers you by helping you understand how to better manage your CKD through the Chronic Kidney Disease Health Education Program.  This program is for eligible members age 18 and older, who are diagnosed with chronic kidney disease.  This no-cost program invites members to take advantage of its valuable resources, including:

  • Health information related to CKD and End Stage Renal Disease (ESRD).
  • Important information about how to help slow down the progression of CKD.
  • Support for care and management of dialysis therapies and treatments.
  • Educational materials that reinforce clinical recommendations.
  • Assessment with a registered nurse to help monitor your progress.
  • Tools to help you communicate better with your health care professionals.
  • Personal dietician to review your individual nutritional needs.
  • Information about innovations in treatment and nationally recognized guidelines to help you manage your condition.
  • Valuable resource information  including community programs, support groups and web-based educational resources.

The goal of this program is to help you reach your optimal health care goals and adhere to the treatment plan established by your physician.
