Twelve million Americans age 18 and older are estimated to have Chronic Obstructive Pulmonary Disease (COPD).  However, close to 24 million adults in the U.S. have evidence of impaired lung function, indicating an under-diagnosis of COPD.*

 

Horizon Blue Cross Blue Shield of New Jersey empowers you by helping you understand how to better manage your COPD through the Chronic Obstructive Pulmonary Disease Health Education Program.  This program is for eligible members age 30 and older, who are diagnosed with COPD.  This no-cost program invites members to take advantage of its valuable resources, including: 

  • Toll-free support line staffed by registered nurses specializing in COPD care.
  • Quarterly educational mailings about COPD.
  • Personal dietician to review your individual nutritional needs.
  • Information about innovations in treatment and nationally recognized guidelines to help you manage your COPD.
  • Valuable resource information including community programs, support groups and web-based educational resources.

The goal of this program is to help you reach your optimal health care goals and to help you adhere to the treatment plan established by your physician.
