Heart disease and stroke, the principle components of cardiovascular disease, are the first and third leading causes of death for both men and women in the United States – nearly 40 percent of all deaths in the country.  More than 927,000 Americans die of cardiovascular disease each year; this amounts to one death every 34 seconds.

 

Although these preventable conditions are more common among people age 65 years or older, the number of sudden deaths from heart disease among people age 15 to 34 has increased.*

 

Horizon Blue Cross Blue Shield of New Jersey empowers you by helping you understand how to better manage your coronary artery disease through the Coronary Artery Disease Health Education Program.  This program is available for eligible members age 18 and older, who are diagnosed with coronary artery disease.  This no-cost program invites members to take advantage of its valuable resources, including: 

  • Toll-free support line staffed by registered nurses specializing in coronary artery disease care.
  • Quarterly educational mailings about coronary artery disease.
  • Personal dietician to review your individual nutritional needs.
  • Information about innovations in treatment and nationally recognized guidelines to help you manage your chronic condition.
  • Valuable resource informationincluding community programs, support groups and web-based educational resources.

The goal of this program is to help you reach your optimal health care goals and adhere to the treatment plan established by your physician.
