If you have diabetes, did you know that every percentage point drop in A1c blood test results reduces your risk of eye, kidney and nerve disease by 40 percent?  Many people with type 2 diabetes can control their blood glucose by following a healthy meal plan, exercising regularly, losing excess weight and taking oral medications.*

 

Horizon Blue Cross Blue Shield of New Jersey empowers you by helping you understand how to better manage your diabetes through the Diabetes Health Education Program.  This no-cost program invites eligible members of all ages to take advantage of its valuable resources, including: 

  • Toll-free support line staffed by registered nurses specializing in diabetes care.  A professional Care Specialist will work directly with you in managing your diabetes.  They will give you support on making healthy lifestyle changes, such as quitting smoking, performing daily exercise, eating a healthy diet and maintaining a proper weight.
  • Education on appropriate tests and screenings you should receive; for example, A1c, LDL, Microalbuminuria and Dilated Retinal Exam.
  • Quarterly educational mailings about diabetes.
  • Personal dietician to review your individual nutritional needs.
  • Information about innovations in treatment and nationally recognized guidelines to help you manage your diabetes.
  • Valuable resource informationincluding community programs, support groups and web-based educational resources.

The goal of this program is to help you reach your optimal health care goals and adhere to the treatment plan established by your physician.
