Did you know that currently, about 5 million Americans are living with heart failure, and about 550,000 new cases are diagnosed each year?  Cardiovascular conditions continue to be one of the top 10 reasons for hospitalization.*

 

Horizon Blue Cross Blue Shield of New Jersey empowers you by helping you understand how to better manage your heart failure to improve your health and wellness through the Heart Failure Health Education Program.  This program is for eligible members age 18 and older who are diagnosed with heart failure.  This no-cost program invites members to take advantage of its valuable resources, including: 

  • Toll-free support line staffed by registered nurses specializing in heart failure care.
  • Quarterly educational mailings about heart failure.
  • Personal dietician to review your individual nutritional needs.
  • Information about innovations in treatment and nationally recognized guidelines to help you manage your chronic condition.
  • Valuable resource informationincluding community programs, support groups and web-based educational resources.

The goal of the program is to help you reach your optimal health care goals and adhere to the treatment plan established by your physician.
