What is this program about?

The main goal of Horizon Health Rewards is to reward members for taking an active role in managing their health. This unique program integrates the powerful online tools and resources within My Health Manager, powered by WebMD, with an exciting rewards program. By using online health and wellness activities, points are earned that can be redeemed for a large selection of gift cards, merchandise, travel packages and more. Also, if the member is enrolled in Horizon’s CDH plan and their employer allows, there is an opportunity to deposit reward funds directly into an HSA account.
