Lab Tests

To be covered at the in-network level of benefits, your lab work must be provided by LabCorp or AtlantiCare Clinical Labs.

Your participating doctor may perform a lab test in his/her office and send it to either LabCorp or AtlantiCare Clinical Labs for testing. If you do not use either of these labs, you will be covered at the out-of-network level and have to pay more for your lab work.

LabCorp

Your doctor can refer you directly to a LabCorp Patient Service Center by giving you a LabCorp Requisition Form. To find a LabCorp Patient Service Center near you, please visit www.labcorp.com or call 1-888-LAB-CORP (522-2677).

AtlantiCare Clinical Labs

The LabCorp Requisition Form can be used at AtlantiCare Clinical Labs. AtlantiCare Clinical Labs are located in southern New Jersey in the following locations:

  • 1750 Zion Road, Suite 209, Northfield, NJ 08225

Present the Requisition Form and your Horizon POS ID card at the lab facility. If you have questions or receive a bill for lab work, please call Member Services.
