Certification/Preapproval — Authorization by us for a doctor to provide treatment to our members. Your doctor is given a certification number after this approval is given.

Chiropractic — A specialty focusing on the nonsurgical treatment of neuromuscular skeletal conditions by means of manipulation of the spine and related structures.

Colon/Rectal Surgery — A specialty focusing on the diagnosis and treatment of disorders of the colon, rectum, and anal canal.

Copayment — A specified dollar amount which a member must pay for each medical visit.

Dermatology — A specialty focusing on the diagnosis and treatment of disorders of the skin, hair, and nails.

Dietetics/Nutrition — A specialty focusing on applying nutritional data to regulate the diet of healthy and sick individuals.

Endocrinology — A specialty focusing on the diagnosis and treatment of disorders of the glands.

Endodontics — A specialty branch of dentistry that deals with diagnosis and treatment of diseases of the dental pulp, pulp chamber, and root canals.

Evidence of Coverage — A supplement, which outlines your specific Horizon HMO, benefits, and exclusions. The Evidence of Coverage is used in conjunction with your member handbook.

Family/General Practice — A specialty focusing on providing continuing, comprehensive primary health care for the entire family.

Gastroenterology — A specialty focusing on the diagnosis and treatment of disorders of the digestive system.

General Surgery — A specialty covering a wide variety of surgical procedures.

Genetics — A specialty focusing on the diagnostic and therapeutic procedures for patients with genetic-linked diseases.

Geriatrics — A specialty focusing on the prevention, diagnosis, and treatment of disorders in the elderly.

Gynecology — A specialty focusing on the care and management of the female reproductive system.

Hand and Forearm Surgery — A specialty focusing on the diagnosis and treatment of disorders in the hand and forearm.

Hematology — A specialty focusing on the treatment of diseases of the blood, spleen, and lymph glands.

Infectious Disease — A specialty focusing on the diagnosis, cause, and treatment of contagious disease.

Internal Medicine — A specialty focusing on the diagnosis and nonsurgical treatment of disorders of the internal organs.

Magellen Health Services — A managed mental health and substance abuse (including alcoholism) care company that coordinates and approves all Horizon HMO mental health and substance abuse care benefits.

Medical Emergency — The sudden, unexpected onset, due to illness or injury, of a medical condition that is expected to result in either a threat to life or to an organ, or a body part not returning to full function.

Midwifery — A specialty focusing on the care of women and the fetus during pregnancy and delivery.

Neonatology/Perinatology — A specialty focusing on the diagnosis and treatment of conditions of sick newborn infants.

Nephrology — A specialty focusing on the diagnosis and management, by medication, diet, or dialysis, of malfunctions and diseases of the kidneys.

Neurology — A specialty focusing on the diagnosis and treatment of disorders of the nervous system.

Nonparticipating — Doctors or facilities who do not contract with us to provide care to Horizon HMO members.

Obstetrics — A specialty focusing on the care of women and the fetus during pregnancy and delivery.

Oncology — A specialty focusing on the diagnosis and treatment of cancers or tumors.

Ophthalmology — A specialty focusing on the diagnosis and treatment of disorders of the eye.

Optometry — A specialty focusing on the diagnosis and nonsurgical treatment of disorders of the eye and vision care.

Oral & Maxillofacial Surgery — A specialty branch of dentistry relating to the surgical treatment of the diseases and injuries of the mouth, teeth, and adjacent tissues.

Orthodontics — A specialty branch of dentistry dealing with the correction of the alignment of teeth and the prevention of malocclusion.

Orthopedics — A specialty focusing on the restoration and prevention of form and function of the extremities and associated structures.

Otolaryngology — A specialty focusing on the diagnosis and treatment of disorders of the ear, nose, and throat.

Out-of-Area Urgent Care — Outpatient or out-of-hospital medical care which, as determined by us or an entity designated by us, is rendered outside the service area, and required by an unexpected illness or accidental injury that is not life threatening, but should be treated before the member or dependent returns to the service area.

Participating Physician — A Primary Care Physician (PCP), specialty care physician, or other medical services professional or organization (hospitals, labs, etc.) that are part of the Managed Network; or a Magellan Behavioral Health participating facility or participating practitioner that offers services to Horizon HMO members.

Pediatric Dentistry — A specialty branch of dentistry focusing on the treatment of the dental needs of children.

Pediatrics — A specialty focusing on maintaining health and providing medical care for children from birth to late teens.

Periodontics — A specialty branch of dentistry devoted to the treatment of diseases of the gums and periodontal structures.

Physician — Doctor, health care professional, or general practitioner.

Physical Medicine/Physical Therapy — A specialty focusing on the planning, conduction, and evaluation of a physical therapy program for patients requiring physical rehabilitation.

Plastic Surgery — A specialty focusing on corrective operations to the face, head, and body to restore function as well as appearance.

Podiatry — A specialty focusing on the diagnosis and treatment of disorders and injury of the feet.

Preapproval/Certification — Authorization by us for a doctor to provide treatment to our members. Your doctor is given a certification number after this approval is given.

Primary Care Physician (PCP) — A doctor of medicine (M.D.) or osteopathy (D.O.) specializing in Family Practice, General Practice, Internal Medicine, or Pediatrics. This doctor contracts with us to provide health services and to coordinate overall health care for Horizon HMO members who have selected them.

Prosthodontics — A specialty branch of dentistry pertaining to the restoration and maintenance of oral function by the replacement of missing teeth and associated structures.

Pulmonary Medicine — A specialty focusing on the diagnosis and treatment of disorders of the lungs and airway.

Radiology — A specialty focusing on the use of radiation and other radioactive substances to diagnose and treat disease.

Referral — The written recommendation by your Primary Care Physician for a member to receive care from a different doctor, provider, or facility.

Reproductive Endocrinology — A specialist in Ob/Gyn who is capable of managing complex problems related to reproductive endocrinology and infertility, and whose current professional activity involves the practice of reproductive endocrinology in a setting wherein essential diagnostic and therapeutic resources are available and being used appropriately.

Rheumatology — A specialty focusing on the diagnosis and treatment of disorders of the joints, tendons, muscles, ligaments, and related structures.

Speech Therapy — A specialty focusing on the diagnosis and treatment of speech disorders.

Thoracic Surgery — A specialty focusing on the diagnosis and treatment of disorders of the chest.

Urgent Care — Outpatient or out-of-hospital medical care which, as determined by us or an entity designated by us, is required by an unexpected illness or accidental injury that is not life-threatening, but should be treated by a provider within 24 hours.

Urology — A specialty focusing on the diagnosis and treatment of diseases of the male genital tract and the urinary tract in both males and females.

Vascular Surgery — A specialty focusing on the diagnosis and surgical treatment of disorders of the blood vessels, excluding the heart, lungs, and brain.
