Health and Wellness Education Programs Overview

1.
Who is eligible?
  ASTHMA & CHRONIC OBSTRUCTIVE PULMONARY DISEASE (COPD) HEALTH EDUCATION PROGRAM:
Available for eligible enrollees age 5 and older diagnosed with asthma or members 30 years and older with COPD (COPD includes chronic bronchitis and emphysema) who are enrolled in the Horizon FEHBP Basic and Standard Option Plans. Asthma program effective November 2004. COPD program effective February 2006.

  DIABETES HEALTH EDUCATION PROGRAM:
Available for eligible enrollees age 18 and older, diagnosed with diabetes and who are enrolled in Horizon FEHBP Basic and Standard Option Plans. Effective November 2004.

  CORONARY ARTERY DISEASE HEALTH EDUCATION PROGRAM:
Available for eligible enrollees age 18 and older who have been hospitalized for cardiovascular conditions like Angina (chest pain), Heart attack (AMI - acute myocardial infarction) or had procedures such as CABG (Coronary Artery Bypass Graft Surgery), PTCA (Percutaneous Transluminal Coronary Angioplasty) and Stent. Available for eligible enrollees enrolled in Horizon FEHBP Basic and Standard Option Plans. Effective November 2004.

  END STAGE RENAL DISEASE (ESRD) HEALTH EDUCATION PROGRAM:
All eligible enrollees diagnosed with End Stage Renal Disease (ESRD) age 18 and older who are enrolled in Horizon FEHBP Basic and Standard Option Plans. Effective November 2004.

  HEART FAILURE HEALTH EDUCATION PROGRAM:
All eligible enrollees age 18 and older, diagnosed with Heart Failure, who are enrolled in the Horizon FEHBP Basic and Standard Option Plans. Effective November 2004.

  HEPATITIS C VIRUS (HCV) HEALTH EDUCATION PROGRAM:
Available for eligible enrollees age 18 and older diagnosed with Hepatitis C Virus, who are enrolled in the Horizon FEHBP Basic and Standard Option Plans. Effective February 2006.

  MULTIPLE SCLEROSIS HEALTH EDUCATION PROGRAM:
All eligible enrollees age 18 and older diagnosed with Multiple Sclerosis, who are enrolled in the Horizon FEHBP Basic and Standard Option Plans. Effective February 2006.

  WEIGHT MANAGEMENT HEALTH EDUCATION PROGRAM:
Available for eligible enrollees age 12 and older diagnosed with Obesity, who are enrolled in the Horizon FEHBP Basic and Standard Option Plans. Effective February 2006.

How was I identified for this program?
  You may have been identified either through our confidential review of claims, pharmacy data, your doctor's referral, or our case management activities.
3.
Is the program free?
  Yes, to eligible Horizon FEHBP enrollees (please see #1 above for listing of programs). Participation in the programs will not directly affect your premium or your status with the Horizon FEHBP Plan.
4.
What are the program benefits?
  The program provides tools to help participants with chronic conditions to better manage their health and improve their quality of life in collaboration with their physician's plan of care.
5.
What providers can I see?
  You should continue to choose providers as you usually do under your Horizon FEHBP Plan.

Do you have a list of preferred providers?
  You may search for a provider through our web site www.fepblue.org or provider directory, or contact customer service at 1-800-624-5078.

Who can I contact with questions or to self enroll in a health education program?

8.
My spouse/friend/family member has a chronic condition and would like to participate in this program. Can they?
  Yes, if the individual meets the eligibility mentioned under question #1.
9.
What if I join the program and then decide to not participate?
  Participation in the programs is completely voluntary. At any time, you can contact us at 1-866-697-9696 and disenroll from the program. Customer service will be able to transfer calls to pilot number x59691 while on the call effective 10/29/04.

Are Durable Medical Equipment (DME) and drugs covered?
  You are covered for DME and prescription drugs as outlined in your Service Benefit Plan Brochure.

Is my health information kept confidential?
  Yes. We always work to ensure that all member personal health information remains confidential.
12.
Who will be answering the phone? What hours are the phones available?
  The care specialists within the health education programs are registered nurses, social workers, or dietitians. They will be answering the phone when a program participant calls. Our care specialists will be available Monday through Friday from 8:00 a.m. to 7:00 p.m. Eastern Time, except holidays. You are also able to leave a voicemail message after office hours.
13.
How quickly will the call be returned?
  All calls will be returned within 24 hours from the time the message was left during the business week. If the message is left on Friday after hours, or over the weekend, a return call will be made on Monday of the following week.
14.
How often will I receive information?
  All members who are determined to be eligible to participate in the programs will receive an introductory letter and brochure informing them about the programs and the how participation in the program can help them improve management of their chronic condition. All members will receive quarterly educational materials and high-risk members will receive telephonic education. ESRD (End Stage Renal Disease) members will receive an on-site visit in their dialysis facility by a care specialist if they are treated in a New Jersey facility.
