lists only the most commonly prescribed drugs. In some cases, your benefit plan
may exclude certain prescription drugs. Please refer to your plan documents for
information regarding your pharmacy benefits, limitations or exclusions, or call
member services at the number listed on your Horizon BCBSNJ ID card. If you have
questions about your drug therapy or alternative therapies, please contact your
