Specialty Pharmaceutical Program for the Treatment of Infertility

Horizon BCBSNJ members, including New Jersey State Health Benefits Program and School
Employees' Health Benefits Program members, who use certain prescription drugs associated
with the treatment of infertility and want to use their in-network benefits to minimize
their cost-sharing, are required to obtain these prescription drugs from one of the
participating specialty pharmacies listed below. These pharmacies provide a high level
of service along with prescription drug-specific education and support.
