Heart Failure Health Education Program
Members, 18 years and older, with heart failure are eligible to join the Heart Failure Health Education Program. The program centers around a team of highly trained cardiovascular nurses who can reinforce the plan of care established between the physician and the patient.
All participants enrolled in the program will receive:
- Educational materials focusing on heart-related topics.
- Ongoing clinical assessments to monitor the use of medications and treatment plans.
- Educational materials that reinforce clinical recommendations.
- Assessment tools to help monitor their progress.
- Tools to help them communicate better with health care professionals.
