SHBP/SEHBP Dental
State Health Benefits Program (SHBP) and School Employees’ Health Benefits Program (SEHBP) Employee Dental Plans are available to full-time State employees, full-time employees of a Local employer (when elected by resolution) and to the eligible dependents of these employees. The Employee Dental Plans offer a choice between two types of plans - the Dental Expense Plan or a Dental Plan Organization (DPO). Click here for information on Horizon Dental Choice (HDC), the DPO offered by Horizon Blue Cross Blue Shield of New Jersey. HDC covers 100 percent of all eligible preventive services and most basic dental services with no copayments, maximums or deductible when receiving those services from a primary care dentist in the HDC network. The HDC Plan also covers charges for all eligible major and specialty services with affordable copayment amounts for those services when referred by your primary care dentist to a Horizon PPO Dental specialist. There is no out-of-network benefit. Each member can choose his or her own primary care dentist. This selection can be changed within the network on a monthly basis, effective on the first day of any month by giving Horizon BCBSNJ 15 days notice.
